This google sheet can be used to easily keep track of notes and contact for your students on your caseload. I have found this method of keeping track of my sessions data and notes to be the best method for me. It easy to access from any computer (when using google drive), as well as share spreadsheet with other team members to keep them informed on how the student is doing. I also enjoy having less paperwork to keep track of and less paper wasted!
