This form is used for when students want to abandon a book. The point of this form is to limit students from choosing "not right" books frequently. It's a questioner that is worded in a VERY SAD way for students to feel guilty about abandoning their book. There are 9 guilty worded questions for students to complete before being allowed to abandon their book. The goal is for students to convince their teacher that they have a better option lined up for them!

This form is for when students finish reading a book. The point of this form is for students to PROVE that they have read the book completely. This form asks simple questions about the story elements and their opinion on the book.

Use this resource while recording individual weekly conferences with students. It is an easy way to track what was discussed during the conference, as well as track who you met with each week. The goal is to write down one positive comment to the student and one teaching point for the student to work on for that week.
