I put this onto the back of my students reading logs each week. They have to complete each activity to show they are reading their books. It helps me to maintain accountability both for parents and students.

I made these to use for missing assignments. When a child has not completed something or forgottten to hand it in, they fill one out and leave it in a spot on my desk. It helps me to keep track and I save them in their files to show their parents at conference time!
