Portrait pages are pre-k to 2nd grade specific, and are specific to the 2018-2019 school year. I will upload next year's in 2019!

This set includes 2 surveys: one to send home for parents/families to complete, and the other for the student to complete. Use this for the beginning of a new school year or for new students joining your class. Students may complete student survey on their own or teacher may take dictation depending on different levels! These are a great way to get to know your students and their families!
