This simple sign-in sheet is what I use to ensure that my students are reading books that are leveled according to their lexile. It is specifically titled to support the Scholastic reading program "Reading Counts." Students will write their assigned lexile range at the top and then will be required to only read books that fall within that range. You can have peers check the range, or check it yourself.
