Simple quiz that incorporates several skills and also exposes students to word problems using decimals.

This writing activity allows students to incorporate basic math skills and writing in preparation for testing and to be included in their writing portfolios.

I used this chart with students and they received a sticker at the end of the day if they accomplished their objective. If a student was misbehaving you can document the problem behavior for parents.
