Here is a homework letter that I usually send home when homework assignments are not submitted.

Daily Reading Log Contract is a tool used for Back to School Night and is frequently revisited throughout the school year to have to accountable for their reading at home. The Form is in English and Spanish.

The letter address' the following: Excessive talking Lack of effort Lack of attention Disrespectfulness to others Incomplete or late assignments Missing assignments Poor test scores Excessive detentions Make the template your own by adding your school name, address, and logo.

Here is a file that I used to include with our take home reading book library. The file was kept in a folder that included my students' reading log. A copy was also given to parents.

This file allows your students to reflect on their behavior. The form includes: student reflection, problem, consequences, and signatures.
