Give this survey to find out how your students' opinions about math. This survey will also allow the students to tell you all about themselves. Then, give this survey back to the students at the end of the year to see if their opinions have changed. In trying to go paperless, I created a google form last year. I have my parents and students go to stations during Open House or Meet the Teacher. I have the parents and students sign up for Remind, explore the grading website, pick an additional supplies to donate, find my tutorial times and get some helpful websites.
This form is given to my students during the first week of school. It goes over all the basic classroom information as well as all my classroom procedures.
