Students and teachers can use this form to track what is being read, how well a student is doing on AR tests, and how well a student is reading at his or her grade level. This is a great document to use if you want a student to develop a personal reading goal for a short time or for a whole school year!

I hold Bachelor's Degrees in Music, Elementary Education, and Social Studies. I also hold a Master's Degree in Reading, Writing, and Literacy.
Additional biographical information
I have been the PLC leader for 8th grade English at Falling Creek Middle School in Richmond, Virginia for five years. I have lead the way in reform at our school in order to introduce a Reading Initiative across grade levels and curriculum.
