I really love having a whole week at a glance so I tend to design all my lesson plan forms this way. These are forms I made to document small group and rti interventions that I use with students. I made this to use with a particularly challenging student I had in Kindergarten. He needed a visual schedule of his day and I needed a way to communicate and remember things that were happening during the course of the day. He earned a sticker or smiley face for all areas of the day where he had appropriate behavior, The space on the side I used to jot notes for me and mom.

"Kid Count" is a number book that you can make with your students while learning the numbers. After reading the number book "Mouse Count" by Ellen Stoll Walsh with my students I wanted to do something fun with it. I printed multiple headshots of each of my students and as we learned the numbers we made our pages by gluing the correct number of children in the jar. They drew the bodies to go with the heads. They turned out really cute and the students loved counting their friends in the jars.

This is a daily small group lesson plan form. I type my student's names at the top where I can easily track daily attendance in small group. I use check marks and an "A" for absent. I made this form to assist me in staying on track with my groups and to create a structure and routine that would be targeted and effecient. Time is always the enemy...there just never seems to be enough time with small groups.

I have a great husband who is also a P.E. Teacher, basketball Coach and bus driver. I have 2 children and 5 grandchildren!
