This equipment sheet allows you to mark off any specialized equipment your student may need. I provide copies to staff working with the student so they are aware of any and all equipment.

This simple note is made for Orthopedic Impairment Teachers (OI teachers/specialist) to send home with their students or as a quick note to the student's teacher.
