After students have read the beginning of year documents (such as the class syllabus, supply list, and learning expectations) this Google Form can be used for students to submit their completion assurance. Students can also submit any questions they have about these beginning of year documents. I use this form to collect all data from my students in regards to the beginning of year documents they must read for my math classes. TpT will automatically add this form to your drive.
