Student Quesionnaire/Survey (Get to Know You)
Get to know your student with this aesthetically pleasing, clean and simple, straight-forward, and comprehensive questionnaire or survey.
Collect basic information such as name, age, birth date, and last year's teacher(s). Learn a student's favorite and least favorite subject, as well as why these are their most and least favorite. Also, learn what a student's most memorable moment was from their last year.
All of these questions are posed in kid-friendly language. They also include examples

Teaching style
I like to mix constructivist and traditional methods. I also rely heavily on Columbia Teacher's College literacy methodology: Reader's and Writer's Workshop.
