This Google Form survey is great for getting feedback on your teaching. I use a version of this each year so that I can reflect on my teacher and adapt as needed moving forward. The Google Form is completely editable to tailor for the needs of your class. It is currently set for secondary math classrooms, but could be edited to fit the needs of any class.
