This is a discipline policy template used in the middle school setting. The policy is set and would be ideally used for schools with set interdisciplinary teams. This template uses tiers of disciplinary infractions with set consequences. Tier I: Class, Tier II: Team, Tier III: Office. This is a teacher-created guide for disciplinary actions.

Label to use for Accelerated Reader program. Includes space for student's name, ZPD level, and points goal for marking period. May be used for reading folder, student agenda, etc. for students and teachers to be aware of student's standing in AR at any time.
