This survey was designed to get to know my students during the first week of school. This would also be great for a first day of school activity. This would be helpful to get a good idea of how each individual student feels about their reading and mathematics ability.

This is a unique resource for mathematics. A five minute write is a written reflection for a math concept. I typically use them at the end of a unit. I have included five examples of five minute writes to get you started and a growth chart for students to keep track of the number of words.
