This document is able to be edited to meet your exact needs for content! I am teaching both math and science this year, and I gave my students two versions of this inventory. The one available here is the full length version my students completed for me first. Later in the day, I gave students a second inventory that was shorter and just pertained to math. (I deleted the questions about what last year, and what they were excited/nervous for as they already answered this for me).

Experience

Teaching style
A variety of teaching styles are found withing my classroom... Workshop days revolving around the Math Daily 3, small group problem solving/inquiry, direct instruction mini lessons and student lead discussions around vocabulary instruction.
My own education history
Attended high school in Michigan and Pennsylvania before completing Capital University's (Bexley, ohio) middle childhood education program. Certified 4-9 math and science with my generalist endorsement to teach ELA and social studies 4-6. I also have a minor in mathematics. I completed my masters in school administration at The Ohio State University and have my principal license for the elementary and middle school level.
