I needed a way to get my students engaged in learning and take more autonomy in the classroom so I created this handout that allows students to reflect on how they worked in their "math teams" and self-assess their work.
At the end of the month, the math team with most 5's or stars earned a prize.
So far it has been working so keep our fingers crossed!

Great way to help students self-monitor their work. Instead of the teacher keeping track of feedback given to student, students can keep this sheet in their binders and anytime the teacher interacts or conferences with a student, it is their duty to complete notes.
