I use this document to allow students to rework test items for partial credit after a math assessment. Students are offered a chance to rework the problems after the test to assure they learn from their mistakes and prove their understanding of what went wrong, in order to improve their grades while also increasing their understanding of the specific area of need.
I use these documents to monitor progress throughout the year for my students. I collect progress data and then have much to discuss with the students in our mentoring sessions. These are great for sharing progress data during Parent/Teacher conferences as well.
