I plan to use this goal sheet this year to help my student work on either an academic goal or a behavior goal. My plan is to meet with students each quarter to set a goal together. It can be something as simple as remembering a library book, or something like keeping hands to self during whole group carpet time. I created this poster, when I got sick of the hand-drawn poster that I made. I got the lesson idea from a book, The Daily 5: Fostering Literacy in the Elementary Grades, by Gail Boushey and Joan Moser. The idea is to help beginning writers from getting frustrated when trying to get their ideas on paper. I usually demonstrate how to use this strategy in front of my kiddos, then send them to their desks with their writing journals, and a piece of gum! How fun!

Use this simple chart to help with students who need individual behavior plans. If you need to chunk the day, so that there is a reward more immediately, than at the end of the day. Edit it to fit your schedule/the child's needs. Print them out daily, and post for the student to see.
I use these subject headers as the headers for my focus wall/objectives. The Powerpoint shows each subject, (reading, phonics, writing, math, science, social studies) in 6 colors. You can crop the slide to pick the color that you would like. The colors are: pink, orange, yellow, green, turquoise, lime, purple.

I suggest placing this reference chart somewhere near your Word Wall or some where close to your writing anchor charts. This is a nice reference, so that students aren't constantly asking for your assistance.
The first chart references a personal word wall, which I use in place of a word wall in my class. The second chart references the word wall.
