I created this evaluation to help pinpoint what basic skills my struggling math learners need help with. This evaluation helps narrow down which number sense skills that a student is struggling with. The evaluation questions test knowledge of counting patterns, place value, and equations, among other skills. There are 14 questions on the evaluation. The teacher copy identifies the skills that are tested for each question and space for observation notes.

This can be used to practice counting by ones, fives, and tens. The first page helps them practice using the hundred chart format, challenging them to use what they know to fill it in with some of it blacked out. The second page requires them to figure out the pattern first by looking if the tens, ones, or both tens and ones places are changing.
