This form is filled out when students are absent. I have one student in charge of collecting their materials and writing what they missed. When the students return, they will find this on their desk!

Class reward coupons. Great for upper grades. No border so it makes it easy to cut out and copy. Color and black and white versions of coupons. This responsibility form helps students and you keep track of missing work. They fill out one of these forms to turn in if they do not have their assignment to turn in. This helps you keep track of what missing work they have and helps the students get it in. Also, this is a great resource to provide documentation of missing work for parents.

Reading log tracker. This can be used in class or at home. Students track the book they are reading and what page they start on and end on. This helps keep track of how fast they are getting through books and if they are actually spending silent reading time reading their book.
