This is a rubric for grading a summary. I use it for my 5th grade classroom, but it could be appropriate for any upper grade classroom. My current assignment is a summary of "Earthquake Terror" from Houghton Mifflin. This rubric follows the CA 5th Grade ELA standards.
This is a letter I send home regarding my expectations for accelerated reading and quiz taking. It also includes a brief summary in Spanish. I also attach the AR parent form that includes their zpd level.
As we finish up Theme 1 of Houghton Mifflin, the children are assigned a natural disaster report. We work in class on a sample report on hurricanes, then they choose either volcanoes, earthquakes, tsunamis or tornadoes and they write their own report. This is the assignment and grading key.
Learning patriotic songs is a fifth grade standard in California, so I have created a songbook in booklet style for each of the students. You can arrange it so that it copies back to back to make a booklet.
To use this, I use a pocket chart and label 3x5 cards with the topics and the dollar amount, just like in the real jeopardy game. The kids play as table groups and call out a category and a dollar amount. I collect the card once it has been answered correctly. It is a great activity for review before unit assessments.
I copy enough of these forms to last the year for each child and put them in a pocket portfolio. I fill it out each Friday and it is due on Monday. It correlates with the ticket notices I already have posted.
This is a ticket notice I send home at the end of the week for those students who have accumulated behavior tickets during the week. The parents then sign and return this notice. It comes in English and Spanish.
This is a page for the students to fill out for each selection. It matches the focus wall and is great for reviewing after the selection or using it throughout the selection.

This is a jeopardy style game to play with the students. It works very well as review before a test. I write the categories and the dollar amounts and use a pocket chart to make it look like the real jeopardy. The kids have a lot of fun reviewing this way.
When a child is absent, another child will pull a pocket portfolio and list all classroom activities and homework assignments on this sheet. When the child returns to school, they have a list of everything they missed in class and all of the homework is in this portfolio. They then turn in their homework along with this sheet the following day. I have also included an absences log for the classroom.
