This collaboration log is specifically useful for special education teachers. I created this as an easy-to-use system to record my collaboration notes for committee, staff, teacher, and grade-level meetings as well as communication with parents and service providers. I print and keep blank pages of this document in a binder to organize my collaboration. Each page contains a section to indicate the person who you communicated with, the date of collaboration, method of communication (meeting, phon
