I used this for students to sign up to take an AR test. They managed themselves by going in the order that they signed up. When they completed an AR test they placed a checkmark in the box by their name.
I posted this version of the form on my website. I deleted the names so that my student names were not posted online. However, when I sent this form home with the students, I edited this copy under "Due Dates" section. I listed the days of the week and had the name of each student by a day. You can use this letter to send home when it is time for you to have a PEP meeting with a child's caregiver. In NC when children score a Level 1 or 2 on End of Grade test a PEP is written. During the school year, I had to meet with the caregivers of the students that scored a Level 1 or 2 on EOG during previous school year. I used a folder with a clear view pocket in front to insert this page into. This just stated to families how to review student work that is sent home. You could also tape this to the cover of a plain folder or insert into one of the folder pockets. You can edit info. for student name, teacher name, & grade level.
This is something I use when my students are absent. It saves time because I don't have to write down makeup work more than once. I just use this pre-made template and type it once. Then I print out as many copies as I need. I open my template each morning and as the day goes I fill in what makeup work absent students will need to do.
