Student Behavior Plan (Actions/Rewards and Actions/Consequences Format) This plan follows an if/then format where the action is listed and a reward or consequence follows. The form includes a sample and a blank PDF file that you can customize. This form serves as documentation for PST or IEP Referrals if completed/followed properly. Designed to be used with Scott Foresman Science Grade 5, but could be used with other series. When we use this during STEM stations, the students use the iPad to research the topics as well as use the textbook. Great way to incorporate Strategic Teaching strategies into Science content area instruction.

This chart is perfect to use when a whole class switches together to another teacher's room. Good for "Teaching Families," team teaching, or STEM/Humanities split classes. The chart travels with the class on a clipboard. Warnings and bathroom breaks recorded so that the same student is not getting 1st warnings from all his/her teachers. Ensures consistent discipline across all classes/teachers.

Use this behavior chart for students who have behavior intervention plans or any student who needs reminders throughout to day to monitor his/her behavior. There are columns for on-task behavior and completion of work. The first example is what I use in my class, but there are several other examples and blank charts as well. Fifth Grade Science Alabama COS Posters Posters can be used to teach the Alabama Course of Study Science Objectives for 5th grade. I printed these out, hung on my classroom wall, and put a check mark on the poster when we covered that objective. It's a great way to see the progress you're making throughout the year.
