This is a weekly or bi-weekly progress report to use when communicating with parents about their student's academic progress. There is a space for: Work Habits Effort/Attitude Areas of Concern Areas of Praise Behavior Academic Concerns There is also a space for comments for the teacher or parent to write in. There is also a space for a signature. There are two reports on one page, copy and cut.

This is a 22 page packet with pages for students to fill out for their Data Notebook. Pages included: Title Page, Class Mission Statement, Quarterly Behavior Goals, Quarterly Academic Goals, Quarterly Writing Sample Reflection Sheets, Spelling and Math Facts Data Recording Sheets, & Weekly Reflection Letter.

This is a great from for student to use when reflecting on their week at school. It is easy to use and kid friendly. There is section they can rate themselves and questions they answer. They can also write a brief letter to their parent(s) or guardian about their week. There is a space for parents/guardians to reply. I keep these in my student's data notebook. 3 pages = 1 title page, 1 page of credits and the form.

This is a great and simple reflection sheet for a student's behavior. I use this for mid-quarter progress reports and quarterly report cards. Students circle which quarter they are reflecting on. Then using the scale on the bottom of the form, they circle and rate themselves between one and four on how they think they did.

Behavior clip chart posters - Supper, Great Choices, Good Choices, Ready to Learn, Think About It, Teacher's Choice and Parent Contact. Made with stripes backgrounds. You can print on card stock and mount onto construction paper in the corresponding color. Then laminate for durability. This is nine pages (2 = tittle and credit page) & seven chart pages. This set of posters allows students to see expectations depending on the type of learning taking place.
