I use this template to plan my DI groups and activities on a weekly basis. I can also create one for you that is subject specific or use for one day if you are a content area teacher who sees a different class every period, just let me know. I will upload it at the same cost. This plan is excellent documentation to refer back to when pulling data for RTI or during principal evalulations where they ask for evidence of your planning and preparation or how you are differentiating your instruction!
I use this form for the last two years to guide my open house conversations. I have uploaded it in Word so that you may edit the graphic to show your grade level,and change or add/delete any subject headings.
