This is an Academic Evaluation Report Template for Special Ed Teachers who give the Woodcock-Johnson Tests of Achievement as part of an Initial or Triennial Special Education Evaluation. If you use a different type of assessment, this template could be used as an outline for creating a report more specific to your needs.
This document includes a table and bell-curve representation of Classifications for Scores on Normed Assessments. This document can be used to help interpret scores on the Woodcock-Johnson Tests of Achievement, as well as other assessments that use a mean of 100 and standard deviation of 15.
