I have used this survey to gain an understanding of what my middle school students think about the school climate. It has also helped me understand what they think their parents expect of them and what they think of their peers. This document is fully editable.

This is a sheet I print and use at the beginning of the year for my students. I put their lock combination on the sheet to help them remember the directions.
