This teacher questionnaire is a great tool for teachers to fill out and give to their district occupational therapist or a member of the IEP/RTI team. This teacher questionnaire is also used by occupational therapists. The OT would have the students classroom teacher fill out the questionnaire as part of an OT evaluation/re-evaluation or progress report.
