This is a form created for speech-language therapists to use when communicating with parents about RtI (response to intervention) for their child in any of the areas of speech and language. The first page is a brief description of the teacher referral (check-box style), and parent permission or refusal (to be returned to school). The second page is a more in-depth explanation of RtI for parent information.

This is a packet that can be printed and copied for speech room folders. Included: *cover page (name, days/time of speech sessions, goals, speech room behavior expectations) and a stamp/sticker chart with random-interval prize earnings.

This form helps me document that all necessary staff members have received a copy of the IEP at a glance form (and/or a full copy of the IEP if I mark it that way). I staple this 1/4 page to the top of the IEP at a Glance, the teacher initials and dates it, then put the 1/4 page in my mailbox (they keep the Glance for their files). Challenge students to earn a Dollar Challenge dollar by completing 100 of their goal behaviors. This could be 100 correct speech sounds in 1 session for an artic student, 100 correct wh-question answers in a month for a language student, etc. When they earn the Dollar Challenge Dollar, have them sign their name on a printed dollar and add it to your Dollar Challenge bulletin board. Packet includes: counter graph up to 100, artic graph up to 10 trials, and parent note home saying they earned it!
