This basic parent notification of student behavior letter is an excellent addition to any classroom management system. In my classroom once a student's conduct has reached 60% or below I send home this letter to notify the parents. Parents must sign the letter and when returned I keep them for reference and documentation purposes.
This form can be used to compile pertinent information about a students progress that you would like to discuss with parents before a conference. It includes space to document academic grades, i-ready, mibels, dibels, Star/AR progress as well as student work habits and classroom behavior.
This template can be used to notate your recommended learning interventions for students and shared with parents. This basic form includes an area to document areas of deficiency in ELA/Reading, Math, Scocial Studies, and Science. It also includes a colunn to notate your suggested interventions and resources. This form also served as great documentation for students within the RTI process.
