Use this very simple punch-card to keep track of how many times your students use the bathroom and get a drink throughout the week. There are 10 punch dots per week, and the dots are larger than a hole punch, so you can clearly see which dots have been punched. The sheet contains 15 individual cards. For best results, copy onto card-stock and allow students to color/decorate their own punch cards.
