Food Allergy Student Information Sheet

Created by
ReadTeachWrite
One in 13 children have food allergies, and that's about 2 children per classroom! As a teacher and food allergy mom, I created this student food allergy information sheet for teachers to quickly access information about their student's allergies. The teacher can fill it out or it can be sent home for parents to fill out. It can be placed with sub plans so that the sub has information readily accessible as well.

About the store

Experience

I've taught for 8 years as a LA/SS teacher and general education teacher. I have taught 4th and 5th grade. I also have teaching and tutoring experience in 3rd and 6th grade. I'm a food allergy advocate and heavily involved in the food allergy community.

Teaching style

I believe in teaching the whole child. I believe that all students can thrive when they are given the appropriate tools and support.

My own education history

I obtained my Master's degree in Education as a reading specialist and have a Bachelor's degree in Elementary Education with a middle school LA/SS endorsement. I am certified to teach students in kindergarten through fifth grade in all subjects. In addition, I am also certified to teach LA and SS in grades 6-8.

Additional biographical information

Although teaching is one of my passions, my other passion includes advocating for those with food allergies. I have experience as a mother to two food allergic children. I'm committed to making school a safer and more inclusive place for children with food allergies.
