All you need to do is add in the photo and crop to size. This is a PDF, so you will need to open the document into a PDF Editable program to make edits. This document is for general education teachers to be able to document the strategies that are utilizing before special education services are needed. This allows for the teachers to track data, over a four week period. There is a team meeting page where participants will agree upon the intervention and then a second page where the teacher will track data on the intervention to see if it is working or not.
