These book club or library club permission slips are a great way to keep track of club membership. It is also a good way to protect yourself from any liability issues, so I recommend keeping the signed forms in a file drawer. This looks best in color, but black and white is just as effective.

Due to confusion regarding how and when students were getting home after library club, my principal requested that I create a document that parents would fill out and sign that would state how their child would be getting home. I keep these on file, in case students aren't sure if they should walk home or wait for a ride.
