These book club or library club permission slips are a great way to keep track of club membership. It is also a good way to protect yourself from any liability issues, so I recommend keeping the signed forms in a file drawer. This looks best in color, but black and white is just as effective.

This flyer was created to promote the Scholastic Book Fair and Online Book Fair. I sent them home with students and posted this on my website. It received a lot of compliments and brought more attention to the library's website than before.

Due to confusion regarding how and when students were getting home after library club, my principal requested that I create a document that parents would fill out and sign that would state how their child would be getting home. I keep these on file, in case students aren't sure if they should walk home or wait for a ride.

This simple invitation is great for sharing information with staff in schools or libraries with more than one break room or time clock.
