This item is the form I use to guide my parent-teacher conferences. On it I take notes about each student's strengths and areas of improvement for math, reading, writing, and behavior/work habits. I use the same form for the entire year and update it as the year progresses.
On the back I keep data of student scores for reading levels, math tests, and district assessments.

Includes a spot to do a quick running record. Usually I write the word and how it is read incorrectly so that I can quickly track errors. There is also a place to record any notes about comprehension and how they are doing in the targeted skill area (such as problem and solution).
