Tired of losing the books in your classroom library??? I was too!! I created this form for students to use when they check out one of the books from my room. I copy several of these at a time and put them on a clipboard with a pencil attached to a piece of yarn and tied to the metal clip ring at the top. I used a cup hook to hang it from my classroom bookshelf. I have managed to keep up with the books in my room using this method more than any other method I have tried in the past! Enjoy!
