This is a great packet to hand to your students that have an IEP. They can fill it out or have their parents fill it out and return it to you. It includes the basic information as well as sections titled: Things I am good at in school, Things I need support to be good at in school, Things I want you to know about me, IEP eligibility, goals and objectives, accommodations and modifications, therapy times per week, and When I grow up I want to.
Have your students' parents asked for a communication log? Many IEP's have a communication log written into the accommodations section and it must be followed. Look no further for a weekly log! This can be copied and pasted into your own document to list your student's exact classes if they are different than the ones I've included.
How does a team know if an accommodation or modification is needed or successful? Without data, the team is unable to definitively know which ones are needed or successful. This data collection tool makes it easy to maintain data on Accommodations and Modifications.
Use these questions to open up the conversation between the entire team before, during and after the IEP meeting. Remember, we don’t have to accomplish everything in one stressful meeting. We can work together and discuss these details ongoing... as a team.
This simple sheet can be used for both gen ed and special ed teachers to take notes on regarding a student they share. Then this sheet can be taken into meetings or just given to the other teacher to look over and use as discussion points. This can be a great way to communicate with one another and it is documentation that you are working with one another. You can each sign and date at the end of each week or at the end of the quarter. Reprint as many as you need.
