This is a form to use with any subject that allows the students to evaluate themselves and their teachers to better understand what the students liked and disliked about the teaching and also what they could have done better if they didn't do well on a test. It helps the students better understand what they can do to improve and also helps the teacher to better understand the learning styles of the students.
