I give this to my students a few times a year when they can self-assess their social responsibility skills. I normally use it to promote discussion. I also have them do this before they do report cards and I send it home with them. You can also use a different colour pen and mark them as well if you want. I only mark it, if I disagree with them in some area.

They also have to pick a muscle in the human body and they have to give three stretches or exercises to strengthen and stretch that muscle.
