These cards are for teachers to use to help track student understanding and accommodations. I use this in my classroom by stapling the filled out card to the specific assignment for the student. The specific card will provide students with a few directions to help them understand what they are supposed to do. The cards can also provide information and feedback to other teachers including intervention specialist. This pdf document comes with six cards per page. I print each one on a specific col

The first form is the request to retest. It is used by the teacher to keep a record of students who want to be retested on quizzes and test. I use this form to keep track of when the intervention took place as well as when the actual retesting occured. The second form is the request to assess. It is used by the student to inform the teacher of when he/she was absent and what test they need to make up. It provides the student and teacher with the opportunity to talk about the time table for compl

Experience

I graduated from Marian University in Indianapolis Indiana and completed student teaching in the Indianapolis public schools and charter schools. I will be teaching my 9th year this year as a high school mathematics teacher. I completed my masters of education at University of Cincinnati and I was able to earn my administration licensure as well. I have taught various courses at the high school level including applied algebra, algebra I, geometry, honors geometry, probability and statistics, business mathematics, and Project Lead the Way courses.

Teaching style

I want to provide my students with many in class practice problems so that they can ask any type of questions that may occur during homework problems. I want notes to be structured but challenging to all students. I like to have notes and in class problems all in one packet directed by verbal explanations or a presentation. I think that if students have a structured layout and visually appealing design, they will be able to complete the homework with confidence and be able to ask detailed questions.

My own education history

I earned a bachelors in secondary education bachelors and mathematics from Marian University. I earned my masters from University of Cincinnati in education with a concentration on a principal licensure.

Additional biographical information

I have a passion for teaching and getting to know my students and their abilities as an individual. I believe that if a teacher knows their students, then there is a greater opportunity for success academically and socially for each student!
