A one-page, quick and dirty way to take notes during my annual review meetings. I kept it pretty general so that it would be useful to many. We have all our annual reviews in the spring, so everything I write on these pages is for the next school year. When I’m finished, I stick this page in in the front of the student’s speech file and review at the end of the summer as a refresher of the most important parts of the meeting.
