This chart comes in handy when looking at the crucial information that is in multiple students' IEPs. There is a place for the student's name, their current reading level, their required service minutes for specially designed instruction (SDI), and their accommodations and modifications. In the footnote section, you can change the date of the most recent update so you and your colleagues know which version is most accurate. I update mine each time Progress Reports go out and each time there is a
