This is a parent questionnaire for students who are bilingual (Spanish and English). Both English and Spanish versions are included. You can gain information from parents such as: what age did the child begin speaking, what language were his/her first words in, and which language is the child most proficient in, including other valuable information to guide you in your bilingual assessment.
