When to track?  During independent play activities including outside, motor room, carpet place etc.  The number of opportunities will be somewhat objective  each undesirable activity present by a peer or therapist, opportunity for instruction to peer or therapist, interruption of therapist or other negative behavior in attempt to receive attention etc.  The number of prompts will be each time you needed to provide an additional verbal or gestural prompt after the original Sd was stated or missed
