Observation Sheet to use when documentation is needed. It has blank places to fill-in the behavior in which you want to observe, occurrences, and total. (i.e. can make tally makes in occurrence box each time student blurts out an answer). The form also has a place to document student name, date, time, subject, and addition notes.
Blank template allows you to fill in each class period or subject, leave comments for each class, and choose a smile straight or frown face for each. It is a great visual with younger general or special education students.

A chart that only shows K and W on one page and the L on a separate page. It can be copied front to back or on different pages. I created this when my students thought they had to fill-out the complete chart each time they were given it to use. By separating the L onto the back or on a different page, it helped the students understand the KWL chart a lot more.
I use this paper with the Wilson Reading Program to get my students used to writing without the line picture cues. It has all 4 lines and has a shaded area between the rows to help elevate confusion when the "worm" line is not being used.
