This is a behavior contract that I use when students are having trouble with certain behaviors in the art classroom! Students and parents sign it & this is a formal notice that consequences will happen for breach of contract. It is always a good idea to have the student participate by adding a goal for their behavior as well!

For disciplinary purposes for the classroom. Use for incidents with students who need to work on behavior that has to do with respect & responsibility. This also can be used as documentation for detentions and/or consequences.

Think sheet is for students who have not followed classroom expectations and you need to have them reflect on their behavior. Customizable to your classroom expectations and rules! This focuses on students behavior and improving character. Can also be used for documentation for parents about what happened as well as holding the student accountable.

An easy set-up to write out all the details of a practice! Can be used for any sport!

If you have clients or do photography on the side, having a print release form is essential! You can also use this for Photography Classes to teach about Copyrights.
