These are easily printable (and editable) in Google Sheets. I've used the Lexend font to make them easily readable from across the room, but feel free to customize to your heart's content! This is a quick little survey you can use in a 1 on 1 conference with a students to help you understand why they miss school, why they come to school, and why they are tardy. I usually write the number of absences and tardies above the student's name, so I have a frame of reference when questioning them. Younger grades should have this filled out by the teacher or other adult, but upper grades could do this independently.

I searched EVERYWHERE trying to find fact practice sheets or tests that had the number being tested in 1st and 2nd positions…I COULDN’T FIND ONE!!! So, I MADE ONE!!! These fact sheets have all the possibilities, and no repeats (except for doubles facts in the “Addition Facts to 9” set-I couldn’t leave it with 19 problems…it just seemed WRONG!!!).
